As a condition of enrollment, the parents grant permission for their student to be taught the health content in the Health Education and/or Health and Physical Education course per the HB-88 “Healthy Youth Act of 2009” and/or for their student to submit brief videos showing mastery of Physical Education (P.E.) skills or have authorized staff of the school/district to record/submit brief videos showing mastery of Physical Education course skills through the NCVPS learning management system. We also have an NCVPS Health and Physical Education Prerequisites, Strategies, and Best Practices Checklist for schools, students, and parents. We also have a Health and Physical Education Spanish version.
In the past, our permission form required parents to grant consent for the student to complete the reproductive health and safety education content as a condition of enrollment. The forms have been updated so that parents/guardians may grant permission to enroll and WITHHOLD consent for the topics above. If a parent/guardian selects that option, the enrolling school must immediately notify NCVPS by completing this Qualtrics form. Dr. Soni is monitoring form submissions and will alert the applicable teachers to exempt the student from the reproductive health content.
